What is Chlamydia?

Chlamydia (Chlamydia trachomatis) is one of the most common sexually transmitted infections in Japan. Many infected individuals are asymptomatic and remain unaware of their condition, leading to a risk of unknowingly spreading the infection to partners.

Main Symptoms

For men

  • Urethral discharge
  • Urinary discomfort/pain
  • Swelling or pain in the testicles (epididymitis)
  • Often asymptomatic

For women

  • Increased or abnormal vaginal discharge
  • Irregular bleeding
  • Lower abdominal pain
  • Often asymptomatic, can sometimes lead to infertility

Pharyngeal infection from oral sex

  • Sore throat/discomfort
  • Mostly asymptomatic

Transmission Routes and Precautions

Chlamydia is transmitted through mucous membrane contact (sexual intercourse, oral sex, anal sex). While condoms can offer some protection, they do not cover all areas of mucous membrane contact. Since chlamydia is often asymptomatic, regular testing is crucial after engaging in risky behaviors.

What is Syphilis?

Syphilis (Treponema pallidum) is a sexually transmitted infection that has been rapidly increasing in Japan recently. The symptoms appear and disappear in cycles as the infection progresses, and if left untreated, it can cause serious effects throughout the body.

Stages of Syphilis Progression

Stage Period Main Symptoms
Primary 3–6 weeks after infection A painless sore (chancre) appears at the site of infection (genitals, lips, anus, etc.). It often goes unnoticed as it disappears naturally.
Secondary 3 months – 3 years after infection Rash (syphilitic roseola) all over the body, including palms and soles, fever, fatigue, swollen lymph nodes.
Latent After secondary stage Symptoms disappear, but bacteria continue to multiply in the body.
Tertiary/Quaternary Several years after infection Severe damage to the heart, blood vessels, nerves, brain, etc.

If you experience any of these, please consult us as soon as possible:

  • Painless lump on genitals, lips, or anus
  • Rash on palms or soles
  • Had sex with multiple partners
  • Partner was diagnosed with syphilis

Can it be transmitted through kissing?

Syphilis can be transmitted through kissing alone. If there are syphilitic lesions on the lips or in the mouth, there is a risk of infection through mucous membrane contact during kissing. Since it cannot be said that "it's okay just because it's kissing," please consult us as soon as possible if you have any concerning symptoms.

What is Gonorrhea (Gonococcal Infection)?

Gonorrhea (Neisseria gonorrhoeae) is a sexually transmitted infection caused by gonococcus, and like chlamydia, it is common in Japan. While symptoms tend to be severe, antibiotic-resistant gonorrhea, which is difficult to treat with antibiotics, has become a problem in recent years.

Main Symptoms

For Men

  • Severe pain and burning sensation during urination
  • Purulent discharge from the urethra
  • If left untreated, can progress to epididymitis and prostatitis

For Women

  • Purulent vaginal discharge
  • Pain during urination
  • Often asymptomatic; if left untreated, can lead to pelvic inflammatory disease and infertility

Pharyngeal infection (due to oral sex)

  • Sore throat/discomfort
  • Mostly asymptomatic, making it hard to notice

If you experience any of these, please consult us as soon as possible:

  • Severe pain or burning sensation during urination
  • Purulent discharge from the urethra or vagina
  • Had oral sex without a condom
  • Partner was diagnosed with gonorrhea

What is DoxyPEP (Doxycycline Post-Exposure Prophylaxis)?

DoxyPEP (Doxycycline Post-Exposure Prophylaxis) is a method to prevent sexually transmitted infections such as chlamydia, syphilis, and gonorrhea by taking doxycycline (an antibiotic) after sexual intercourse.

According to research, DoxyPEP has been reported to have a preventative effect of approximately 87% against syphilis, 88% against chlamydia, and 55% against gonorrhea. It is an effective option for emergency prevention in situations where unprotected sex has occurred or where there is a high risk of STI.

Statement Regarding Unapproved Drugs

The doxycycline (Vibramycin) used at our clinic for STI prevention (DoxyPEP) is an off-label use as it is not approved for this indication in Japan. It is imported by our clinic through a formal process with the Ministry of Health, Labour and Welfare, and is provided as a self-pay service not covered by insurance. Clarification on the availability of approved drugs in Japan: Yes

Consider DoxyPEP if any of these apply:

  • Had unprotected sex
  • Condom broke or came off during sex
  • Had sex with multiple partners
  • Uncertain about partner's STI status

Preventative Effects of DoxyPEP
